- It will facilitate our work if you do
this at least one day before your appointment.
- 2. If you are unable to e-mail the
completed form, print out the

History Questionnaire. Fill it out in long hand
and bring it with you to your appointment.

Form that corresponds with the service you are receiving.
Bring the appropriate signed consent
form with you to your first appointment.
